How to Take the Abortion Pill: Your Step-by-Step Guide

How does the abortion pill work?

The first pill, mifepristone, blocks a hormone necessary for a pregnancy to continue to develop. Most people will feel fine after taking mifepristone and can resume normal activity until they take the misoprostol. You may see a small amount of bleeding after taking mifepristone, but this is normal.

The second set of pills, misoprostol, cause cramping and bleeding from the vagina so that the pregnancy will leave your body (similar to a miscarriage). It is usually taken at home within 72 hours after taking mifepristone, the first pill.

How do I take the abortion pill?

Step 1 Mifepristone:

Take the first pill when you are ready to begin

When you are ready to begin the medication abortion, take one (1) mifepristone pill, it’s a single pill in a blister pack. Swallow it with water just like any other pill. Remember to make a note of the date and time you took the mifepristone. Most people will feel fine after taking the mifepristone, and you can resume normal activity until you take the Misoprostol. You may see a small amount of bleeding. This is normal, and you should complete Step 2 with Misoprostol even if you see bleeding after taking just the mifepristone.

Step 2 Misoprostol:

Up to 72 hrs later take the 2nd set of pills

Misoprostol is not swallowed like most other medications. There are two ways to take it: vaginally or buccally (by holding it in your mouth cheeks and letting it dissolve). You may choose whichever way is most comfortable for you. You can take misoprostol at any time after taking the mifepristone, from immediately after mifepristone up to 72 hours later. It is most effective when taken between 6 and 48 hours after taking mifepristone.

Your choice of method impacts when you should take misoprostol. If you want to take misoprostol less than 24 hours after taking mifepristone, it should be taken vaginally. If you want to take misoprostol bucally (in your cheeks), you should wait at least 24 hours after taking mifepristone.

Step 3 wait for the medication to take effect:

The abortion begins within 1-4 hours after taking Misoprostol.

Many people will start to have cramping and bleeding within 1 to 4 hours after taking the misoprostol. In some people, it can take up to 24 hours for bleeding and cramping to start. Both are normal.

Note: If it has been at least 4-6 hours after you took the initial dose of misoprostol and you have not had any bleeding, you may want to consider using the optional second dose of misoprostol through the same method you took the first dose. A second dose of misoprostol is included in your medication package (4 additional pills). If your pregnancy has progressed past 9 weeks, we suggest you use the second dose of misoprostol within 4-6 hours after the first dose, even if you had bleeding. If you still do not have bleeding, like a menstrual period or heavier, after the second dose of misoprostol, please take the 3rd dose the same way you did with the second dose 4 hours after the second dose.

Please note, if your pregnancy was less than 9 weeks, you may not have received a 3rd dose of misoprostol.

What to Expect After Taking the Abortion Pill

Many people will start to have cramping and bleeding within 1 to 4 hours after taking the misoprostol. In some people, it can take up to 24 hours for bleeding and cramping to start. Both are normal. Here’s what you can expect:

  • Bleeding is expected and may be like a period or heavier. Blood clots are also normal. Most people experience several hours of heavier bleeding that quickly decreases once they have passed the pregnancy from their body. Some people do not bleed again until their next period starts in about 4-6 weeks. Others may bleed on and off until their next period starts. These are all normal possibilities. As long as you are not bleeding through more than two pads an hour for more than two hours, or passing blood clots larger than a lemon, or are dizzy or lightheaded, then it is considered normal.
  • Cramping is the pain you feel as your uterus contracts to pass the pregnancy. Your cramps may be strong at first but will get better as the abortion is completed. Some things that can help the cramping are taking your pain medication, putting a heating pad or hot water bottle on your belly, taking a warm shower, sitting on the toilet, or having someone rub your back.
  • Chills and fever can occur. You may feel cold or have chills on the day you take the misoprostol. It is also possible to feel hot and sweaty or even have a mild fever that goes away after 24 hours. Please call us or your healthcare professional right away if you have a fever 100.4°F or higher more than 24 hours after taking misoprostol (or anytime in the two weeks after taking misoprostol).
  • Nausea, vomiting, and diarrhea may happen. These are normal side effects of the medication and should go away within 24 hours. If these symptoms last more than 2 days or you feel they are severe, please call us or your healthcare provider.
  • Tiredness is normal for 1 or 2 days. You should be back to normal soon.
